家長會員申請／續會表格 Membership Application Form / Renewal Form
皇仁書院家長教師會全年均接受現屆就讀皇仁書院的學生家長登記入會。皇仁書院家長教師會的會籍以每位家長為單位，每位家長需填寫入會表格，並連同會費親身或經郵寄交回本會。
Parents or guardians of all students studying in Queen’s College can apply to become a member of The Queen’s College Parents-Teachers’ Association (QCPTA) throughout the year. The membership of the QCPTA is based on per person basis. Each parent is required to fill in the membership form and return it together with the membership fee to the QCPTA in person or by mail.
